Keloidectomy
What is a keloidectomy?
A keloidectomy is the surgical excision of keloid scars, which are abnormal and unsightly tissue growths. While most surgeons can cut out the keloid, there’s still a possibility of recurrence. It's important to note that surgery is often combined with various other treatments when keloids have not responded to other therapies, providing a comprehensive and thorough approach to keloid management that aims to address all aspects of the condition.
There are various types of keloidectomy, such as:
- Traditional keloidectomy is the surgical resection of keloids along with a healthy surrounding margin of tissue.
- Core excision of the keloid involves surgical resection of the inside of the keloid, using a skin flap to conceal the defect. The centre of the keloid is removed, leaving a rim of tissue intact. This procedure reduces the risk of keloid recurrence.
- Intralesional excision of the keloid is surgery to eradicate the entire keloid while leaving the residual rim of tissue intact. The procedure aims to diminish the size of the keloid without interfering with the healthy, adjacent skin. The incisions are made within the borders of the scar.
- Keloid fillet flap surgery uses skin directly from the keloid scar to rebuild the area once the surgeon removes the scar. Therefore, the surgeon prepares to remove the inside of the keloid scar and a superficial layer of skin from the scar. The remaining flap is then used to seal the area. The advantages of the fillet flap are vast since it optimises the cosmetic results.
- Ligature involves the use of a surgical thread that may be wrapped around the keloid. The thread cuts into the keloid, which may cause it to suddenly drop off.
